Only header shown when view header/footer selected due to no footer select.
Cause:
The footer is not selected.
Solution:
Click 'Switch Between Header and Footer' on the Header and Footer toolbar.
1) Select the 'View' menu and select 'Header and Footer'. (The first Page Header and the Header and Footer toolbar appear.)
